Remote Patient Monitoring

Vivify remote patient monitoring, with home kits, data feeds and care pathways run for you

Overview

What you are buying

Moraph resells, implements and runs the Vivify Health remote patient monitoring platform for health systems, medical groups and health plans. We configure care pathways and alert thresholds your clinicians approve, connect home devices such as scales, blood pressure cuffs and pulse oximeters, and keep readings flowing into the care team's dashboard and your EHR. Your clinicians review readings and decide on care; Moraph runs the platform, the data feeds and the kit logistics behind them.

The problem it solves

Remote monitoring programs often start well and then struggle with the work around the clinical part. Kits go out without anyone tracking whether they were set up, devices stop sending readings and nobody notices for days, alert thresholds are copied from a vendor default rather than agreed by clinicians, and readings sit in a separate portal instead of the EHR. Billing for monitoring depends on records of enrollment, consent, readings and time spent that are hard to pull together.

  • Patients receive kits but some never send a reading
  • Devices that stop transmitting are found late, if at all
  • Alert thresholds and care pathways are not formally approved by clinicians
  • Enrollment, consent, reading days and care time are hard to evidence for billing

Who uses it

  • Medical directors and clinical leads for chronic care and post-discharge programs
  • Care managers and nurses who review readings and contact patients
  • Program coordinators who enroll patients and manage kits
  • Revenue cycle teams who bill for remote monitoring services
  • IT and EHR teams who connect the platform to clinical systems

Requirements it supports

HIPAA Privacy Rule (45 CFR Part 160 and Part 164, Subparts A and E)HIPAA Security Rule (45 CFR Part 164, Subpart C)HIPAA Breach Notification Rule (45 CFR 164.400 to 164.414)Medicare Physician Fee Schedule remote physiologic monitoring codes (CPT 99453, 99454, 99457 and 99458)21st Century Cures Act information blocking rules (45 CFR Part 171)Telephone Consumer Protection Act (47 U.S.C. 227), for automated patient calls and texts

The product supports these requirements; meeting them remains your organization’s responsibility.

A short walkthrough

Illustrative screens of how Moraph runs the service, with fictional sample data. They are not screens of Vivify itself, which belongs to its vendor.

  1. Step 1

    The enrollment and kit tracker

Every referred patient has a row from consent to first reading, so nobody is left with a kit they never set up.

  2. Step 2

    The pathway approval

Each pathway's devices, questions and thresholds are written down and approved by a named clinician before they go live.

Each pathway's devices, questions and thresholds are written down and approved by a named clinician before they go live.
  3. Step 3

    A device that stopped sending

Moraph spots patients whose readings have stopped and follows up until data is flowing again.

Workflows

The principal workflows

  1. Enroll

Patients are referred, consented and enrolled on the care pathway their clinician selects, and a kit with the right devices is sent or handed over.

    • Referral received from the clinician or care management team
    • Coordinator confirms eligibility and records the patient's consent
    • Patient enrolled on the approved care pathway in Vivify
    • Kit with the specified devices ordered and shipped or issued in clinic
    • Setup confirmed when the first reading arrives
  2. Monitor

Readings from home devices reach the care team's dashboard, where Vivify applies the pathway's thresholds and surfaces readings outside range for clinical review.

    • Devices send readings through the patient's tablet or phone app
    • Vivify checks readings against the pathway thresholds
    • Out-of-range readings and missed readings appear on the care team dashboard
    • Care manager reviews and contacts the patient according to protocol
    • Action and time spent recorded on the patient's record
  3. Keep data flowing

Moraph watches the technical side: devices that have stopped sending, failed EHR messages and connection problems, and works with patients and your team to fix them.

    • Daily check of patients with no readings for the period your protocol sets
    • Coordinator or Moraph support contacts the patient about the device
    • Device replaced or reconfigured where needed
    • EHR interface errors reviewed and resent
    • Recurring problems raised in the service review
  4. Close and report

Patients are discharged from the program when the clinician decides, kits are returned, and the records needed for reporting and billing are complete.

    • Clinician decides to discharge the patient from monitoring
    • Kit return arranged and devices recorded as returned or lost
    • Program summary sent to the EHR
    • Monthly records of readings and care time prepared for revenue cycle review
    • Program results reviewed with the medical director

How access is controlled

  • Clinical decisions stay with your clinicians. The platform flags readings against thresholds your clinicians approved; care managers and clinicians decide what to do.
  • Moraph engineers apply changes; your organization approves them. Changes to pathways and thresholds always have a clinical approver, and nobody approves a change they requested themselves when a second approver is named.
  • Moraph staff see patient information only as far as technical support and data feed work require, using named individual accounts with the least rights the service needs.
  • Roles are agreed during implementation and can be limited by clinic, program or care team. Patients use the patient app or tablet and never see staff screens.

Features

What it does

Connected home devices

Vivify receives readings from supported Bluetooth devices such as weight scales, blood pressure cuffs, pulse oximeters, thermometers and glucometers, through a tablet or the patient's own phone.

Care pathways

Condition-specific pathways combine device readings with daily questions and education, configured to protocols your clinicians approve.

Clinician-approved thresholds

Alert thresholds for each pathway, and any patient-specific changes, are set and recorded with the approving clinician.

Care team dashboard

Vivify shows the care team which patients have readings out of range or missed readings, so they can work the highest-priority patients first.

Video and messaging

Care managers can contact patients through the platform's video visits and messaging where your program uses them.

EHR integration

Readings, alerts and program summaries are sent to your EHR through the interface agreed during implementation.

Kit logistics

Moraph coordinates kit orders, shipping, setup support, returns and device replacement with the fulfilment arrangement agreed for your program.

Transmission monitoring

Moraph checks daily for patients and devices that have stopped sending readings and starts follow-up.

Enrollment and consent records

Each patient's enrollment, consent, pathway and discharge are recorded, ready for audit and billing review.

Time and reading records

Reading days and care team time are recorded per patient per month, to support your revenue cycle team's billing review.

Patient technical support

Patients get help with device setup and connection problems from Moraph support or your team, as agreed.

Controlled changes

Changes to pathways, thresholds and settings each have a request, a clinical approver and a record.

Security and privacy

How your data is protected

Business associate agreement

Moraph signs a business associate agreement with your organization because the service involves PHI, and Vivify's own agreement covers the platform.

Named Moraph staff

Moraph staff work with individual accounts, the least rights the service needs and multi-factor authentication; there are no shared accounts.

Role-based access

Care teams see their own patients, coordinators see enrollment and kits, and Moraph sees what technical support requires.

Complete audit trail

The platform's access logs and Moraph's change and ticket records show who did what and when.

Encryption and keys

Service records Moraph holds are encrypted, with keys Moraph staff cannot use without explicit, time-bound authorization.

People decide

Thresholds flag readings for review; clinicians decide on care, and Moraph tooling never contacts patients about their health.

Company-wide controls, independent audits (SOC 2 Type II and ISO/IEC 27001) and data residency are described in our Trust Center, where you can request the evidence pack.

Reporting

What you can report on

Exports, access and evidence

  • Reports are produced by Moraph from the platform's program data and from Moraph's ticket, kit and change records, and reviewed with you each month.
  • Billing support reports show the records behind each month. Your revenue cycle team decides what is billed and checks it against current payer rules.
  • Reports respect each viewer's role: care managers see their own patients, and summary reports for leadership can be produced without patient identifiers.
  • Reports can be exported to CSV, and summary reports to PDF, for committees, internal audit and payer reviews.
  • Report contents and frequency are agreed during implementation; additional views can be requested through the service review.

Deployment

Where and how it runs

Licences through Moraph

Moraph resells the Vivify subscription and runs it as one service, subject to Vivify's terms.

Your existing Vivify program

If you already license Vivify, Moraph reviews pathways, thresholds and integrations and takes over operation.

Co-managed

Your team keeps some tasks, such as enrollment or patient technical support, and Moraph runs the rest; the split is written into the responsibilities for the service.

Security controls, independent audits and data residency are set out in our Trust Center. Engagement models and what drives a quotation are on Working with Moraph.

Implementation

What a rollout involves

The general phases are on Working with Moraph. For this product, they involve the following.

Program design

We work with your medical director on the conditions in scope, eligibility, patient numbers, care team staffing and the protocol for responding to alerts.

Pathways and thresholds

Pathways, questions, education and default thresholds are drafted from your protocols and approved by your clinicians before build.

Devices and kits

Device types for each pathway and the kit fulfilment, setup and return arrangements are agreed and tested.

EHR integration

The interface to your EHR is built and tested with your analysts, including where readings and summaries appear.

Pilot

A first group of patients is enrolled with extra support, and the protocol, thresholds and kit process are adjusted with your team.

Handover to operations

Change process, approvers, support routes, escalation contacts and reporting are agreed, and the program moves into day-to-day operation.

Limitations

What it does not do

Worth knowing before you evaluate it.

  • Moraph is not Vivify Health. Vivify operates the platform, and Vivify's own service commitments, roadmap, maintenance and outages apply.
  • Vivify licences are subject to Vivify's terms. Devices and kits are priced separately.
  • Remote monitoring is not an emergency service. Patients must be told how to get urgent help, and readings are reviewed during the hours your program sets.
  • Moraph does not review readings for clinical purposes, contact patients about their health or make care decisions; your clinicians do.
  • Moraph does not decide what is billed or submit claims. Billing rules for remote monitoring change, and your revenue cycle team checks them.
  • Monitoring depends on patients using their devices and on home connectivity. Some patients will need a cellular-connected kit or extra support.
  • Only devices the platform supports can send readings automatically; others need manual entry.
  • Moraph does not make privacy or breach notification decisions; your privacy officer does.

Evaluation checklist

What to test in a demonstration or pilot

Questions worth putting to us, and to any other vendor you are considering.

  1. Walk a sample patient from referral through consent, kit delivery and first reading
  2. Review the proposed pathway and thresholds with the clinicians who will approve them
  3. Check how a device that stops sending is detected and who follows up
  4. Test the EHR interface with fictional patients and confirm where readings appear
  5. Review a sample billing support report with your revenue cycle team
  6. Confirm the split of responsibilities between your care team, Moraph and Vivify

Questions

Frequently asked questions

Do we buy Vivify from Moraph or from Vivify Health?

Either. Moraph can resell the subscription, or run a program you already license. In both cases Vivify's terms apply to the licence.

Does Moraph watch our patients' readings?

No. Your care team reviews readings and alerts. Moraph watches the technical side, such as devices that stop sending and interface errors, and tells your team.

Does Moraph sign a business associate agreement?

Yes. The service involves PHI, so Moraph signs a business associate agreement with your organization. Vivify's own agreement covers the platform.

Will this make our monitoring billable?

Moraph prepares the records behind each month, such as consent, reading days and care time. Whether a service is billable depends on payer rules and your documentation, and your revenue cycle team decides.

How is it priced?

Vivify licences are priced under Vivify's terms, devices and kits separately, and the managed service and implementation are quoted separately. See Working with Moraph for what drives a quotation.

What happens if the platform is down?

Vivify's own service commitments and incident process apply. Moraph tells you when an incident affects monitoring, and your program's downtime protocol covers patient contact in the meantime.
