Antibiotic Stewardship Tracker

See which antibiotic orders need a second look, act on them, and evidence your stewardship program

Overview

What you are buying

The Antibiotic Stewardship Tracker compares antibiotic orders with culture and susceptibility results, and puts the patients who may need a change of therapy in front of your stewardship pharmacists and physicians. Recommendations to prescribers, their responses and the reasons are recorded in one place. Use and resistance data are summarized by unit and prescriber for your stewardship committee and prepared for your NHSN reporting.

The problem it solves

Stewardship teams usually find their patients by running EHR reports each morning, cross-checking microbiology results by hand and keeping interventions in a spreadsheet. Bug-drug mismatches and long courses of empiric therapy can be missed between reports, recommendations to prescribers are made by phone or page and rarely recorded consistently, and when a surveyor or the committee asks what the program achieved, someone assembles the numbers by hand.

  • Antibiotic orders and culture results live in different systems and are matched by hand
  • Patients who need review are found by daily reports rather than as results arrive
  • Recommendations and prescriber responses are recorded inconsistently, if at all
  • Use, resistance and intervention data are assembled by hand for committees and surveyors

Who uses it

  • Antimicrobial stewardship pharmacists and physician leads in hospitals and health systems
  • Infectious diseases physicians who advise on therapy
  • Infection prevention and microbiology teams who share resistance data
  • Quality, pharmacy and therapeutics, and medical staff committees who oversee the program
  • Accreditation and compliance staff who prepare for surveys

Workflows

The principal workflows

  1. Detect

Antibiotic orders, administrations and microbiology results flow in from your EHR and laboratory system, and rules you configure flag patients whose therapy may need review.

    • Orders, administrations and culture and susceptibility results arrive by HL7 or FHIR interface
    • Each new result is matched with the patient's active antibiotic orders
    • Rules flag bug-drug mismatches, possible de-escalation, duplicate coverage, IV-to-oral candidates and restricted agents
    • Flagged patients join the review queue with the reason they were flagged
    • Flags already reviewed are suppressed until something changes
  2. Review

A stewardship pharmacist or physician works the queue, with the patient's antibiotic history, cultures, susceptibilities and relevant labs on one screen.

    • Queue sorted by flag type, unit and time since flagged
    • Reviewer opens the patient and checks therapy against results and local guidelines
    • Days of therapy and time on the current agent shown alongside
    • Review closed with no action, or a recommendation drafted
    • Patients can be scheduled for follow-up review
  3. Intervene

Recommendations to prescribers, such as de-escalation, stopping, changing the dose or switching to oral, are recorded with the prescriber's response and the reason.

    • Recommendation recorded with type, agent and rationale
    • Recommendation communicated to the prescriber by your usual route and logged
    • Prescriber response recorded as accepted, modified or declined, with a reason
    • Order changes in the EHR matched back to the recommendation
    • Restricted-agent approvals recorded with who approved and why
  4. Measure and report

Days of therapy, intervention outcomes and resistance patterns are summarized for the committee, and AUR data is prepared for NHSN submission by your team.

    • Days of therapy calculated by agent, unit and period
    • Intervention acceptance summarized by type, unit and prescriber group
    • Cumulative antibiogram prepared from susceptibility results for microbiology to review
    • AUR files prepared for your team to validate and submit to NHSN
    • Committee reports produced on the schedule you set

How access is controlled

  • Restricted-agent approvals and rule changes are separated from day-to-day review: pharmacists record and request, and the physician lead or a delegated approver decides.
  • Roles are configured with you during implementation. You can rename them, create custom roles, and limit them by facility, unit or service, so each team sees only the patients it is responsible for.
  • Every permission change and every view of a patient record is recorded in the audit trail, and access review reports list each person's roles for periodic certification.
  • Staff sign in through your identity provider, and roles can follow its groups. Surveyors and auditors get temporary read-only access with a defined scope and an end date, rather than shared accounts.

Exports, access and evidence

  • Every report can be exported to CSV, and summary and evidence reports to PDF. Reports can be scheduled to arrive by email for the people entitled to see them.
  • Each report can be filtered and the filters saved, so pharmacists, the physician lead and committee members can keep their own views. Reports that need a different shape can be built in your own tools from the data described below.
  • Flag, review, intervention and use data is available through the Moraph Work API, and can be delivered to your data warehouse or business intelligence tool alongside other quality data.
  • Reports respect each viewer's role: a prescriber sees only their own patients and service, the stewardship team sees the program, and surveyors can be given read-only access to reports and the audit trail.
  • Committee and survey reports can be run as of a past date and give the same result each time. Running and exporting a report is itself recorded in the audit trail.

Deployment

Where and how it runs

Moraph cloud

Hosted and operated by Moraph in the United States region. United Kingdom and Australia regions are available for organizations outside the US.

Dedicated environment

A single-tenant environment for health systems whose policies require isolation, scoped in the proposal.

With a managed service

Optionally run alongside Moraph's managed integration engine for interface monitoring and support.

Security controls, independent audits and data residency are set out in our Trust Center. Engagement models and what drives a quotation are on Working with Moraph.

Implementation

What a rollout involves

The general phases are on Working with Moraph. For this product, they involve the following.

Interface build

We connect to your EHR, laboratory and pharmacy systems and map orders, administrations and results. We need interface specifications and a contact from your integration team.

Rules and guidelines

Review rules, restricted agents and time-out points are configured to your local guidelines with your physician lead and stewardship pharmacists, and each rule is documented.

Validation against history

Rules are run against a period of past data and the flagged patients reviewed with your team before any rule is switched on.

Measures and AUR

Days of therapy and AUR location mapping are set up and checked against your existing figures with your NHSN users.

Workflow and roles

Queues, intervention types, response reasons, approval steps and roles are configured to match your program's procedures.

Training and go-live

Pharmacists, physicians and committee users are trained on their own workflows, and existing intervention records can be imported by agreement.

Limitations

What it does not do

Worth knowing before you evaluate it.

  • The Tracker does not change orders. Prescribers change therapy in the EHR, and the Tracker records recommendations and responses.
  • It is decision support for your stewardship team, not a substitute for clinical judgment. Flags are prompts for review, and each recommendation is made by a clinician.
  • Flags depend on timely, complete order, administration and microbiology feeds. Delayed or missing results mean delayed or missing flags.
  • It does not submit to NHSN itself. Your team reviews and uploads the AUR files it prepares.
  • Review rules reflect your local guidelines, which your program sets and maintains; the Tracker does not supply treatment guidelines.
  • It is designed for inpatient and hospital-based programs. Outpatient and long-term care stewardship are not included.
  • It is not a laboratory information system or an infection surveillance system, although it uses and shares their data.
  • Published antibiograms are your microbiology and pharmacy teams' responsibility; the Tracker prepares the data for their review.

Evaluation checklist

What to test in a demonstration or pilot

Questions worth putting to us, and to any other vendor you are considering.

  1. Run the review rules against a sample of your own de-identified orders and cultures and compare the flags with what your team found
  2. Ask a stewardship pharmacist to work a flag from queue to recorded intervention
  3. Check that days of therapy match the figures you currently report
  4. Review the AUR file preparation with your NHSN users
  5. Walk through a restricted-agent request and its follow-up review
  6. Confirm role separation: who can change rules, approve restricted agents and see which patients

Questions

Frequently asked questions

Does the Tracker change antibiotic orders?

No. It flags patients and records recommendations and responses. Prescribers change orders in the EHR, and the Tracker matches those changes back to the recommendation.

Can we use our own guidelines?

Yes. Review rules, restricted agents and time-out points are configured to your local guidelines during implementation, and your physician lead approves any later change.

Does it submit our AUR data to NHSN?

It prepares AUR files in the format NHSN accepts. Your team reviews them and uploads them to NHSN.

How is it priced?

As a subscription, usually measured by the number of facilities or beds covered, with implementation charged separately. See Working with Moraph for what drives a quotation.

Do you sign a business associate agreement?

Yes. The Tracker handles protected health information, and Moraph signs a business associate agreement before any patient data is loaded.

Is there an accessibility conformance report?

Ask for one through the procurement request form at /how-to-buy/request, naming the version you are evaluating.
